Instructions for first visit
INSURANCE Only the Japanese Insurance (issued by the government,“KOKUMIN-KENKOU-HOKEN”or
the National Health Insurance and by the company, SHAKAI-HOKEN or the Social Insurance) is accepted
and this must be shown on your arrival. If not, all the medical cost must be paid by cash.
If you have been injured on the job, we require a form of verification from your employer (FORM 5) stating
that your injury has been work-related. Otherwise, payment must be made at the time of service.
If you have been injured in the traffic accident, also we need to know which automobile insurance company
will pay the bill for you usually by phone call from that company.
After these paperworks, the doctor will take a comprehensive medical history and perform musculoskeletal
examination, x-rays may be necessary for full evaluation and diagnosis of your complaint. Discussion of a
recommended treatment plan will then follow. In case, diagnosis is equivocal, or your disease status needs
urgent diagnosis and treatment, the doctor will suggest to go to higher level facility and have MRI/CT scan,
or to seek more intensive treatment.
